Rimatil
布西拉明(bucillamine,1)，消炎镇痛药。别名Rimatil, 扑湿胺。化学名 N-(2-巯基-2-甲基丙酰)-L-半胱氨酸为日本参天制药株式会社开发研制的抗类风湿新药,属免疫调节剂,与左旋咪唑、D-青霉胺等比较,具有疗效好、副作用小并可长期给药的优点。

药理
本品对大鼠佐剂关节炎和动物风湿性反应及关节病变，具有显著的抑制作用，其抑制效果与D-青霉胺相同。体外试验发现，本品可使类风湿因子转阴，在Ca++存在下，可抑制γ-球蛋白。本品与D-青霉胺一样，对巨噬细胞可提高吞噬能力，可提高免疫T细胞的分化和机能，但并不影响B细胞的机能。
 　　
药动学  　　
人体口服本品1小时达最高血药浓度，经1小时后从血液中消除，主要代谢产物(S-甲基衍生物)或以原形在24小时内由尿中排出约40％。小鼠经口给予本品后，几乎100％被吸收，分布到除脑以外的所有组织，过一段时间组织中的药物浓度开始下降，但在动脉、气管、皮肤、剑突、膝关节等的结缔组织和软骨组织的消除比较缓慢。与血浆蛋白的结合率为39％～80％。

适应症
慢性类风湿性关节炎(RA)。本品应使用于消炎镇痛药等未获满意疗效的情况。

用法用量
口服，通常成人每次1片(100mg)、一日3次(300mg)，于饭后服。一日最大剂量为300mg。  　　

贮藏
避免高温、高湿贮藏。

禁用
(1)禁用于血液病患者、肾脏病患者、小儿生长期有结缔组织代谢障碍的患者。  　　
(2)慎用于有血液病史、肾脏病史或肝脏病史的患者。  　　
(3)老年患者、手术后患者、骨髓功能低下的患者、全身状态恶化的患者，原则上避免使用本品。  　　
(4)孕妇及小儿用药因尚未确定其安全性，应慎重斟酌。  　　
(5)动物试验，大量给药见胎仔死亡增加及新生发育延迟。  　　
(6)授乳妇女不应使用本品，必须用药时应停止授乳。

给药说明
开始使用本品时应向患者说明其主要副作用及用法、用量等注意点，特别在发现咽痛、发热、紫斑等症状时应迅速就医。  　　
因本品显效迟缓，在本品显效之前应继续并用以前使用的消炎镇痛药等。但本品继续用药6个月仍无效果时应停止给药。  　　
使用本品前应作血液及肝、肾功能检查。服药过程中应仔细观察临床症状，定期进行血、尿及肝肾功能等检查。

不良反应
本品是一种含硫氢基的化合物。在对121例患有风湿性关节炎的病人的多中心、双盲的临床研究中，有23例发生不良反应，包括17例非特异性的粘膜和皮肤反应、6例胃肠失调、3例蛋白尿和1例肝功能障碍。  　　
(1)偶见贫血、粒细胞减少、转氨酶上升等，应停药。  　　
(2)可引起毛细血管脆弱、血栓性静脉炎等。偶见蛋白尿、肾病综合征、肾功能异常，应停药并进行适当处理。
(3)皮疹、瘙痒感，偶见发热、口腔炎等过敏症状，偶见嗜酸粒细胞增加，应停药。  　　
(4)偶见食欲不振、恶心、呕吐、腹泻、便秘等，应减量或停药。  　　
(5)偶见味觉异常、头痛、嗜睡，应减量或停药。  　　
(6)其它：偶见脱毛、指端麻木感、倦怠感、眼痛等。  　　
试用于506个病例，有152例(30.0％)发生不良反应，主要为皮肤、粘膜、消化系统等不良反应。

相互作用
与含金药物合用，可增强不良反应，效果减弱。

Effects of this medicine
This medicine eases inflammation of rheumatoid arthritis by helping abnormal immunity return to normal. It may take 1 to 3 months to start for the medicine to take effect.
It is usually used to treat rheumatoid arthritis.

Before using this medicine, tell your doctor/pharmacist
If you ever experienced any allergic reaction (itch, rash etc.) to any medicine.
If you are a patient with blood disease/history of blood disease, kidney disease/history of kidney disease, or liver disease.
If you just underwent surgery.
If your general condition is deteriorated.
If you are pregnant or breastfeeding.
If you are using any other prescription or nonprescription (OTC) medicine. (It is important to reduce the risk of drug interactions.)
 
Dosage regimen (proper use of this medicine)
Your dosage regimen is:(order of your doctor)
General dosage regimen for adults: Take 1 tablet (100mg of the active ingredient), at a time, 3 times a day, after meals. Your doctor may adjust the dose for your age and condition. After certain effects are seen, daily dose may be changed to 1 to 3 tablets (100 to 300mg). However, the maximum dose is 3 tablets (300mg) a day. Strictly follow the instructions of your doctor/pharmacist.
Missed dose: Skip the missed dose and continue your regular dosing schedule. DO NOT take a double dose to make up for the missed dose.
Overdose: If you took too much of this medicine (more than ordered), check with your doctor/pharmacist.
Others: Do not stop taking this medicine or change your dose without the instructions of your doctor.

Side effects of this medicine
Common side effects are reported as below. If any of them occurs, check with your doctor/pharmacist: rash, itch, proteinuria (dark urine), canker sores/abnormal feeling inside mouth.
In rare cases, symptoms described below may be the sign of side effects indicated in brackets [ ]. If they occur, stop taking your medicine and check with your doctor immediately.
erythema, respiratory distress, dizziness[shock, anaphylactoid symptoms]
fever, sore throat, bleeding tendency[agranulocytosis, thrombocytopenia, other blood disease]
fever, cough, respiratory distress[interstitial pneumonitis, pulmonary fibrosis, other lung disease]
decreased urination output, swelling[acute renal failure, nephrotic syndrome]
loss of appetite, fatigue, yellowing of the skin or eyes[liver function disorder, jaundice]
Other side effects not listed here may occur. Check with your doctor/pharmacist if you have any worrisome symptom. 

Storage and other information
Keep out the reach of children. Store away from heat, humidity and direct sunlight.
Discard the remainder. Do not store them. Ask your pharmacist how to discard.
