药品类别：抗高血压病药 
 
药理药动 
为中枢性α2受体激动剂．其作用与可乐定相似，但其作用较可乐定为弱，因而在降压有效剂量时，中枢性不良反应也小．降压时心率减慢，但心输出量一般并不减少．对肾小球滤过率无明显影响．口服吸收迅速、完全．在血浆中与蛋白结合率亦低．大部分药物被肝灭活．Ｔ1/2为21小时。
[药理作用]本品为中枢α2受体激动药，作用于延脑及脑干血管运动中枢，减少交感神经向周围血管的输出，向心脏输出的影响较弱。本品作用与可乐定相似，但较轻，维持时间较长。
 
药动学
本品口服生物利用度约为80％，消除半减期14～17h。静注后消除半减期与口服基本一致。表观分布容积6L／kg，血浆蛋白结合率为71.6％。约60％～70％的本品经肝脏代谢，其余经肾小管主动分泌后由尿液排出体外。总清除率为414ml/min，肾清除率约占一半，为210ml／min。饮食对本品药物动力学影响不大。
 
适应症
用于治疗中度至重度高血压．可单用或与利尿药合用。
本品主要用于轻、中度高血压病，其疗效与可乐定或α甲基多巴相同，也可用于妊娠毒血症或高血压危象的治疗。本品一般作为三线药物，常与利尿剂合用。
 
用法用量 
口服,开始一次0.5-1mg,一日1次,睡前服.以后逐渐增量至一日3mg.
[用法及用量]口服，每次1～3mg，每日睡前服，通常从小剂量开始。突然停药可发生撤药症状(多发生于停药后2～7日内)，表现为血浆及尿中儿茶酚胺增加，出现神经质或焦虑以及血压反跳至治疗前水平以上。
[剂型与规格]片剂：1mg／片，2mg／片。
 
不良反应 
口干,便秘,嗜睡,乏力,心动过缓,幻听,幻视.个别病例头痛,头晕.可引起恶心,呕吐,食欲不振,腹胀,腹泻.性功能障碍(或性欲下降),水钠潴留.长期使用突然停药可引起轻度停药综合征,不安,震颤.
与可乐定相似，一般较轻，且在用药过程中减轻或消失。主要有口干、眩晕、乏力、便秘或腹泻、头痛、失眠、阳痿、心动徐缓、心悸、恶心、呕吐、抑郁、视觉模糊等。

禁忌症 
对本品过敏者及12岁以下儿童禁用。
 
药物相互作用 
本品增加中枢神经系统抑制药物的镇静作用。与具肝酶诱导作用的药物合用可能使本品消除半减期缩短及血药浓度下降。

Tenex
Other Name: Guanfacine

Category: Antihypertensive

Uses of Tenex:
Tenex belongs to the general class of medicines called antihypertensives. It is used to treat high blood pressure (hypertension) but that may also be helpful in reducing ADHD symptoms. Like another antihypertensive drug, Catapres etc. (Clonidine), Tenex regulates levels of the neurotransmitter norepinephrine and may be effective in reducing ADHD symptoms alone or in combination with stimulants. They also may be used as an alternative to stimulant medication, especially in the 30% of children who cannot tolerate stimulants and in children with conduct problems. However, antihypertensives may not improve attention deficit or distractibility. Tenex may also reduce symptoms of aggression and reduce the insomnia associated with stimulants.

Tenex works by controlling nerve impulses along certain nerve pathways. As a result, it relaxes blood vessels so that blood passes through them more easily. This helps to lower blood pressure.

This medication is sometimes prescribed for other uses; ask your doctor or pharmacist for more information.

Tenex for ADHD and Tics disorders like Tourette's syndrome
Tenex is pharmacologically similar to clonidine. In a controlled trial of children with ADHD plus tics like Tourette's syndrome, Tenex improved symptoms in both domains, though the improvements in attention fell short of what is generally seen with standard medications. Side effects are similar to those of clonidine, though Tenex is less sedating.

Research published in the Journal of Psychiatry (July, 2001) concluded that Tenex appears to be a safe and effective treatment for children with tic disorders and ADHD. In the study, Tenex was associated with a mean improvement of 37% in the total score on a teacher-rated ADHD Rating Scale, compared to 8% improvement for placebo after 8 weeks of treatment.

How to Take Tenex:
Take this medicine as directed. It can be taken with or without food. Do not stop taking this medicine abruptly without consulting with your doctor.

Side Effects of Tenex:
Some common side effects reported with this medicine include:
difficulty breathing
closing of your throat
swelling of your lips, tongue, or face
hives
depression
very slow heart rate (fewer than 60 beats per minute)
unusually high or low blood pressure (fainting, a severe headache, flushing of your face)
unusual fatigue, dizziness, or tiredness
headache
dry mouth
constipation
insomnia
constipation, nausea, vomiting, or diarrhea

Warnings and precautions for Tenex
Do not take other medicines unless they have been discussed with your doctor . This especially includes over-the-counter (nonprescription) medicines for appetite control, asthma, colds, cough, hay fever, or sinus problems, since they may tend to increase your blood pressure.
Check with your doctor before you stop taking guanfacine. Your doctor may want you to reduce gradually the amount you are taking before stopping completely.
Tenex will add to the effects of alcohol and other CNS depressants (medicines that slow down the nervous system, possibly causing drowsiness). Some examples of CNS depressants are antihistamines or medicine for hay fever, other allergies, or colds; sedatives, tranquilizers, or sleeping medicine; prescription pain medicine or narcotics; barbiturates; medicine for seizures; muscle relaxants; or anesthetics, including some dental anesthetics. Check with your doctor before taking any of the above while you are using this medicine .
Before having any kind of surgery (including dental surgery) or emergency treatment, tell the medical doctor or dentist in charge that you are using this medicine.
Make sure that you have enough guanfacine on hand to last through weekends, holidays, and vacations. You should not miss any doses. You may want to ask your doctor for another written prescription for guanfacine to carry in your wallet or purse. You can then have it filled if you run out when you are away from home.

What to do if you take Overdose of Tenex?
Seek medical attention immediately. For non emergencies, contact your local or regional poison control center.

What to do if you take Missed Dose of Tenex?
Take your next dose as soon as you remember. If it is time for your next dose, skip the missed dose and go back to your regular schedule. Do not take a double dose to make up for a missed one.

How to Store Tenex:
Keep this medication in the container it came in, tightly closed, and out of the reach of children. Store it at room temperature and away from excess heat and moisture (not in the bathroom). Throw away any medication that is outdated or no longer needed. Talk to your pharmacist about the proper disposal of your medication.
