英文药名：Cleanal（ Fudosteine）中文药名：福多司坦片给药说明福多司坦（fudosteine）于2001年10月首次在日本上市，由三菱制药和S.S制药生产。剂型为200mg片剂，商品名分别为：Cleanal TABLETS（三菱制药株式会社），SPELEAR tablets 200（S.S制药株式会社）。福多司坦适用于治疗支气管哮喘、慢性支气管炎、支气管扩张、肺结核、肺气肿、非典型耐酸杆菌感染症、弥漫性支气管炎等慢性呼吸系统疾病的祛痰。福多司坦对慢性呼吸系统疾病有多重药理作用，抑制呼吸道上皮细胞增生，可使痰中海藻糖/唾液酸的比值正常化，改善痰的粘性和弹性，恢复纤毛输送气道分泌液的状态，促进浆液分泌，并有抗炎作用。分类名称一级分类：呼吸系统药物二级分类：祛痰药物三级分类：促进痰液溶解的药物药品英文名 Fudosteine 药品别名弗多司坦、Cleanal、Spelear 药物剂型福多司坦片剂：200mg/片。药理作用本品作用机制如下。 1.气道黏液过度分泌作用的抑制机制，抑制杯状细胞过度形成。 2.黏液修复作用。 3.浆液性气道分泌亢进作用。 4.抗炎症作用。本品祛痰效果为羰甲基半胱氨酸的3倍，而毒副作用小(对大鼠和小鼠LD50＞10g/kg)。本品为具有黏液活性的半胱氨酸衍生物，能改变气道黏液的组成并具有黏液修复性质。对人体肺黏液上皮细胞的研究表明，本品在有效血浆浓度(10μm)时显著减少由ATP或缓激肽(Bradykinin)引起的高分子量复合糖(HMWG)分泌，但对由乙酰甲胆碱(Methacholine)引起的HMWG分泌无效果。药动学对雄狗单剂给予14C标记的本品25mg/kg验证其吸收、代谢和分泌。在血液中峰浓度(Cmax)为23.44μg/ml，达峰时间为3.33h，t1/2为15.8h，药时曲线下面积(AUCo-∞)为567.3(μg·h)/ml。在体外，本品在血细胞中分布为2.5%～9.3%。在狗体内，该值在0.5小时后为1.0%，8小时后为26.9%，24小时后为27.2%。服药后0.5～8h，血浆中的母体化合物为32.7%～49.9%，服药168小时后92.9%的原始剂量从尿中排泄，2.2%由粪中排泄。在血浆中的主要代谢物如下，M1 25.6%～37.6%，M4 7.9%～12.3%，M3 5.9%～12.3%。在尿中，主要代谢物及分布为：M1 55.7%，M3 19.6%，M2 12.6%，M4 5.9%。另一项类似研究评价14C标记的本品在雄性大鼠体内的吸收、分布、代谢和分泌。大鼠口服25mg/(kg·d)共10剂。血液中药物浓度不因重复剂量而改变。与单剂量给药相比，最后一次给药后，Cmax和t1/2均升高约1.8倍。医学全/在线www.med126.com第5剂量、第10剂量0.5小时后，其组织浓度与第1剂量后相似。第24小时组织浓度因重复剂量而升高。放射活性主要蓄积在脂肪和脑脂肪中。本品主要在小肠吸收，狗体内蛋白结合率为0.4%～2.5%。适应证适用于咳嗽，慢性支气管炎，支气管扩张症，尘肺病，肺气肿，非定型抗酸菌症等疾患的祛痰。禁忌证对本品过敏者禁用。不良反应副作用为食欲不振、恶心呕吐、头痛、腹痛、便秘。本品对大鼠及小鼠的急性毒性LD50＞10g/kg。大鼠的长期性研究显示，口服本品0、100mg/(kg·d)、300mg/(kg·d)和900mg/(kg·d)12个月，随后为3个月的恢复期。在研究过程中未见与给药有关的死亡，仅在900mg/(kg·d)组观察到一些临床症状。对大鼠9周的实验表明，对雄性和雌性大鼠生殖行为和胎儿发育的一般毒性无毒剂量为2000mg/(kg·d)。用法用量口服，成人每次400mg，3次/d，饭后口服，可随患者年龄及症状程度适当增减剂量。 Manufacturer's name : Mitsubishi Tanabe Pharma Corporation Information is based on approvals granted in Japan.
