奥沙拉秦 Olsalazine
【异名】奥柳氮钠，地泊坦，AzodisatSodium，Cl14130，Disodiumazobis，Dipentum

【性状】常用其钠盐，白色或类白色结晶性粉末，熔点>300℃。

【药理】本品以活性成分5-氨基水杨酸替代柳氮磺胺吡啶中无活性的磺胺吡啶，即通过偶氮键连接两分子5-氨基水杨酸，提高了疗效，降低了不良反应发生率。5-氨基水杨酸是治疗溃疡性结肠炎的有效成份，但口服5-氨基水杨酸后在小肠被吸收，乙酰化后随尿排出，不能到达结肠部位。本品在胃及小肠中不被吸收也不分解，到达结肠部位后其偶氮桥在细菌作用下断裂，分解为二分子5-氨基水杨酸并作用于结肠炎症粘膜，抑制前列腺素合成，抑制炎症介质白三烯的形成，降低肠壁细胞膜的通透性，减轻肠粘膜水肿。
    本品原形很少被吸收，其分解产物5-氨基水杨酸在结肠部位的浓度大于血清中药物浓度1000倍。溃疡性结肠炎患者口服治疗量(每日1～4g)后，本品和5-氨基水杨酸在管腔内的浓度分别为0.04～0.14g/L和0.8～3.5g/L。人口服15mg/kg后，tmax为1～2小时，Cmax为2～4mg/L，24小时后仍有少量药物存留在血液中。口服剂量的99%到达结肠，全身吸收很少。本品分布容积低，仅6.2±1.5L，蛋白结合率高。
    轻度至严重活动性溃疡性结肠炎患者每日口服本品1～3g，疗程3个月，54%～66%患者临床症状获得改善，28%患者病情获缓解。活动性溃疡性结肠炎患者(包括对柳氮磺胺吡啶过敏和不能耐受的病人)每日口服本品0.75～4g达6个月至2年，临床总改善率相似(58%～65%)。病情缓解后继续服药可维持疗效。63%～88%的患者经6～24个月治疗，病情继续获得缓解。

【应用】用于治疗急、慢性溃疡性结肠炎与节段性回肠炎，并用于缓解期的长期维持治疗。

【用法】口服，治疗开始时每日1g，分次服用，根据患者反应逐渐提高剂量至每日3g，分3～4次服用。儿童为每日20～40mg/kg。长期维持治疗，成人每日1g，分2次服用；儿童每日15～30mg/kg。本品随食物同服。

【注意】
    (1)对水杨酸过敏者、严重肾功能损害者禁用。
    (2)孕妇及哺乳期妇女慎用。
    (3)常见不良反应有腹泻、软便(治疗第1周出现，特别是有长期病史或弥漫性结肠炎患者)、腹部痉挛、头痛、失眠、恶心、消化不良、关节痛、皮疹、头晕等。

【制剂】胶囊剂：每胶囊250mg。

How does it work?
This medicine contains the active ingredient olsalazine sodium, which is a type of medicine called an aminosalicylate. It is used for treating the inflammatory bowel disease, ulcerative colitis.

Olsalazine works by reducing inflammation in the lining of the intestines. It is known to reduce the actions of substances in the body that produce inflammation, however the full mechanism of action of this medicine is not completely understood.

Olsalazine reduces the inflammation in the gut that causes flare-ups of ulcerative colitis. It is therefore used to these control flare-ups, and is also used as a maintenance treatment to prevent further flare-ups.

Warning!
This medicine may rarely cause a decrease in the normal amounts of blood cells in the blood. For this reason you should consult your doctor immediately if you experience any of the following symptoms: unexplained bruising or bleeding, purple spots, sore throat, mouth ulcers, high temperature (fever), feeling tired or general illness. Your doctor may want to take a blood test to check your blood cells.
This medicine may cause diarrhoea if it is taken on an empty stomach. To avoid this you should take the medicine at the end of a meal. Consult your doctor if you are still experiencing diarrhoea despite taking the medicine with food.

This medicine should not be used if you are allergic to one or any of its ingredients. Please inform your doctor or pharmacist if you have previously experienced such an allergy.

If you feel you have experienced an allergic reaction, stop using this medicine and inform your doctor or pharmacist immediately.

Pregnancy and Breastfeeding
Certain medicines should not be used during pregnancy or breastfeeding. However, other medicines may be safely used in pregnancy or breastfeeding providing the benefits to the mother outweigh the risks to the unborn baby. Always inform your doctor if you are pregnant or planning a pregnancy, before using any medicine.
The safety of this medicine in pregnancy has not been established. It should therefore be used with caution during pregnancy, and only if the benefits to the mother outweigh any risks to the foetus. Seek medical advice from your doctor.
It is not known if this medicine passes into breast milk. It should be used with caution in nursing mothers, and only if the benefits to the mother outweigh any risks to the nursing infant. Discuss this with your doctor.
