Yakult Honsha公司的5-HT3受体拈抗剂Sinseron(indisetron)(Ⅰ)在日本上市，用于治疗顺铂和其他抗癌药引起的恶心、呕吐。(Ⅰ)是从日新和杏林两家公司获得特许权的。标准给药方案是每天一片(8mg)。

Indisetron hydrochloride (Sinseron, [Mfg.]) is a new oral antiemetic that is administered once daily. It is an antagonist to the 5-HT3 receptors, which are a subtype of the serotonin (5-hydroxytryptamine: 5-HT) receptors. In phase II and III clinical trials, indisetron prevented vomiting in 67 of 108 patients (62%) for 24 hours after the administration of anticancer agents, including cisplatin. In a phase III clinical trial, it prevented vomiting in 34 of 40 patients (85%) for 24 hours after the administration of anticancer agents, excluding cisplatin. Indisetron is metabolized in the liver, and CYP1A1, CYP2C9, CYP2D6, and CYP3A4 are involved in its metabolism. However, indisetron is unlikely to cause drug interactions at clinical doses because the effective plasma concentrations are lower than those necessary for inhibiting the metabolic enzymes. No drug interaction has been reported. The 5-HT4 receptor, as well as the to 5-HT3 receptor, is involved in causing nausea and vomiting. Indisetron antagonizes 5-HT4 receptors, as well as 5-HT3 receptors, and this characteristic is expected to con-tribute to its clinical efficacy. (author abst.)
