临床药理
长期过量的脂肪及高蛋白的摄入，致使血液中总胆固醇、VLDL胆固醇、甘油三酯、LDL胆固醇增高，HDL胆固醇降低为高脂蛋白血症，并进 步导致动脉粥样硬化、冠心病、高血压 吉非罗齐为非卤化的氯贝丁酯类降血脂药物，它能降低VLDL的合成，增加肝外脂蛋白活性，促进VLDL分解而使甘油三酯减少，并抑制动脉粥样硬化的过程。长期服用吉非罗齐可预防冠心病的发生并降低冠心病患者的危险性。口服吉非罗齐快速有效地在胃肠道吸收，1-2个小时可达血药高峰，在所有的组织器官均有较高浓度，t1/2约1.5小时，66%经尿液排出体外，约6%经粪便排出体外，长期服用无蓄积现象。
适应症
用于原发性和继发性脂蛋白血症，如血胆固醇过高（Ⅱ＊＊ａ型）、血甘油三脂过高（Ⅲ型、Ⅳ型）、混合血脂过高（Ⅱ＊＊ｂ型、Ⅲ型、Ⅳ型）。糖尿病引起的血脂过高，血脂过高引起的黄瘤以及冠心病等。
用法用量
①口服：国外推荐剂量每日１．２ｇ，分２次于早、晚餐前半小时服或遵照医嘱用。国内试用每次３００ｍｇ，每日３次。
②口服一次0.3-0.6g,每日0.6-1.2g 另有一部份病人需增加剂量到期。1.5g/天才有效。
任何疑问,请遵医嘱!
注意事项
1.可有肠胃道反应如腹痛、胃肠不适等，通常不严重而不用停药。
2.偶有一过性无症状ＳＧＴ升高，但多半在停药后可恢复正常。
3.孕妇应慎用。
4.病人应定时作凝血酶原测定。
禁忌
严重肝、肾功能不全患者。
药物相互作用
口服抗凝剂患者服用本品时，其抗凝剂剂量应减半。
规格
胶囊剂：１粒 ３００ｍｇ，6００ｍｇ。
Gemfibrozil - Used for: Treating high blood cholesterol and triglycerides. It is used along with an appropriate diet.
Gemfibrozil is an antihyperlipidemic agent. It works by lowering the blood levels of triglycerides and low-density lipoprotein (LDL) ("bad") cholesterol by reducing their production by the liver. It also increases blood levels of high-density lipoprotein (HDL) ("good") cholesterol.
Pharmacology: Decreases blood levels of triglycerides and VLDL by decreasing their production. Also decreases cholesterol and increases HDL.
Indication: Treatment of hypertriglyceridemia in adult patients with type IV or V hyperlipidemia that presents risk of pancreatitis and does not respond to diet; reduction of coronary heart disease risk in type IIb patients who have low HDL levels (in addition to elevated LDL and triglycerides) and have not responded to other measures.
Gemfibrozil, a fibric acid antilipemic agent similar to clofibrate, is used to treat hyperlipoproteinemia and as a second-line therapy for type IIb hypercholesterolemia. It acts to reduce triglyceride levels, reduce VLDL levels, reduce LDL levels (moderately), and increase HDL levels (moderately).
Gemfibrozil helps lower your cholesterol and triglyceride (fat) levels. Excessive cholesterol and fat in your body can clog up your blood vessels. This can lead to many health problems such as strokes and heart disease, including heart attacks. Gemfibrozil is meant to be taken as part of a complete cholesterol management programme that should include exercise, a healthy diet and lifestyle.
Oral Hyperlipidaemias
Adult: 1.2 g daily in 2 divided doses.
Maintenance: 0.9-1.5 g daily.
Indications: Hyperlipidaemia s.
treatment of people with very high levels of serum triglycerides who are at risk of developing pancreatitis and who do not respond adequately to a strict diet. This drug can also be used to reduce the risk of coronary heart disease in people who have failed to respond to weight loss, diet, exercise, and other triglyceride- or cholesterol-lowering drugs.
Indications: Hyperlipidaemia, lipoprotein abnormalities resistant to diet alone. |
