药理作用
本品为长效苯二氮卓类药物，药理作用与地西泮相似，其镇静副作用很小，更适合用于成人癫痫的辅助治疗。本品对不同病因引起的癫痫患者的部分性发作和全身性发作均有效，但通常作为辅助用药。对可预测癫痫发作的患者(例如妇女月经性癫痫)可采用间歇治疗。间歇方法有助于预防耐受性的产生。
 
药动学
口服易于吸收，1～4h可达血药峰值，85%与血浆蛋白结合。在肝内通过，脱甲基和羟基化作用进行代谢。原药和活性代谢物N-去甲基氯巴占的平均t1/2分别为18h和42h。本品高度亲脂，迅速透过血-脑脊液屏障。原药和代谢物主要随尿排出。
 
适应证
适用于焦虑症和难治性癫痫的治疗。对癫痫复杂性发作继发全身性发作和伦-加综合征效果更佳。
 
禁忌证
1.对本品过敏者、孕妇、哺乳者禁用。
2.严重肝病、急性闭角型青光眼和卟啉症患者禁用。
 
注意事项
如连续应用，其抗惊厥作用逐渐减弱，可采用“放假疗法”，如女性患者，在月经期发作时，可在月经来潮前2～3天开始用药，10天后再用。
 
不良反应
1.参见地西泮。
2.偶有焦躁、抑郁和肌无力。
3.偶有月经性癫痫患者服药后经期推迟的报道。
4.突然停药可出现戒断症状，亦可加剧癫痫发作，故停药应逐渐减量过渡。
5.个例报道，本品可致中毒性表皮坏死松解症。
 
用法用量
1.治疗焦虑症：常用每天20～30mg，分次服，或单次晚间服。
2.用于癫痫的辅助治疗：剂量与用法同治疗焦虑症。
3.3岁或大于3岁儿童：推荐剂量为不超过成人剂量的1/2。
4.老年或体弱患者的推荐剂量为每天10～20mg。
 
药物相应作用
1.参见地西泮。
2.与卡马西平、苯巴比妥、苯妥英钠、丙戊酸合用，本品的血浓度降低，而后者浓度升高。
 
专家点评
该药为广谱抗癫痫药物。但因其抗癫痫机制同氯硝西泮，除出现困倦等副作用外可产生药物依赖性。

Mystan - General Information:
Mystan is a drug which is a benzodiazepine derivative. It has been marketed as an anxiolytic since 1975 and an anticonvulsant since 1984.

Pharmacology:
Mystan is a barbiturate used in combination with acetaminophen or aspirin and caffeine for its sedative and relaxant effects in the treatment of tension headaches, migraines, and pain.

Mystan Interactions
Alcohol (increases bioavailability by 50%), cimetidine, and valproates.

Mystan Contraindications
Myasthenia gravis, sleep apnea, severe liver diseases such as cirrhosis and hepatitis, and respiratory problems.

Mystan Indication: For treatment and management of epilepsy and anxiety disorder.
Mechanism Of Action: Mystan binds at a distinct binding site associated with a Cl- ionopore at the GABA-A receptor, increasing the duration of time for which the Cl- ionopore is open. The post-synaptic inhibitory effect of GABA in the thalamus is prolonged as a result.
Drug Interactions: Clozapine Increased risk of toxicity
Kava Kava increases the effect of the benzodiazepine
Food Interactions: Take without regard to meals.
Alcohol increases clobazam absorption by 50%.
