【英文药名】Xylocaine

【中文药名】利多卡因LIDOCAINE

【别名】赛罗卡因、昔罗卡因、Xylocaine、Lignocaine、Xylotox。

【类别】局麻药、抗心律失常药。

【作用与用途】
本品为酰胺类局麻药及抗心律失常药。
1.用于口咽和气管内表面麻醉。
2.硬膜外阻滞或臂丛、颈丛神经阻滞。
3.抗心律失常:主要是室性心律失常

【用法用量】
1.口咽和气管内表面麻醉用2%~4%溶液,一次不超过100mg。
2.硬膜外阻滞或臂丛神经阻滞用1%~2%溶液,成人一次最大用量400mg。
3.抗室性心律失常,先静脉注射负荷量1mg/kg~2mg/kg,再以0.1%溶液每分钟15μg/kg~50μg/kg的速度持续静脉滴注,每小时不超过100mg。
4.盐酸利多卡因软膏:(1)用于口腔疾病:5%本药涂于口腔内吹干的粘膜表面。(2)治疗瘙痒:2.5%本药供皮肤外用。

【不良反应】
运动系统:毒性反应为纤细的骨骼肌震颤或抽搐。主要表现为烦躁不安或寒颤,严重者可出现肌张力增高,甚至抽搐。也可为全身倦怠感。
循环呼吸系统:超量可引起惊厥及心跳骤停。高浓度大剂量局部麻醉可致呼吸骤停乃至死亡。局部麻醉有致呼吸骤停及心动过缓的报道。常量也有引起呼吸衰竭心搏骤停者。有局部麻醉致惊厥者。
过敏反应:罕见变态反应。浸润麻醉可引起过敏反应而出现皮肤红斑、心慌、气促。有致过敏性休克的报道。 可引起哮喘发作。即使是鼻内注射也可诱发支气管哮喘。
眼科手术麻醉可致暂时性光感消失[1592、1593、1594]。
其他:有致癫痫大发作并一过性偏瘫的报道[105]。在常用剂量下有发生视幻觉者[133]。

【注意事项】
1.本品吸收快,易发生毒性反应,故应用中勿误入血管。
2.加入1/20万肾上腺素作神经阻滞可减少毒性作用发生率,延长作用时效。
3.本品扩散强,毒性与血药浓度相关,一般不宜用作浸润麻醉。
4.严重房室传导阻滞、室内传导阻滞禁用。
5.超量可引起惊厥及心跳骤停。
6.连续静脉滴注时必须观察心电图。

【药物相互作用】
本品应尽量避免与甲氨蝶呤配伍使用,以避免增加氨甲喋呤毒性及减低其药效

GENERIC NAME: LIDOCAINE - INJECTION (LOCAL) (LIE-doh-cane)
BRAND NAME(S): Xylocaine

USES: Injectable lidocaine is used to numb an area before surgery or before another medical procedure.

HOW TO USE: This medication is injected into the body (e.g., into a muscle or under the skin) as directed by your doctor. The dosage and location of the injection depends on your condition and response to the drug. Before using, check this product visually for particles or discoloration. If either is present, do not use the liquid. Learn all preparation and usage instructions in the product package. If any of the information is unclear, consult your doctor or pharmacist. Learn how to store and discard needles and medical supplies safely. Consult your pharmacist.

SIDE EFFECTS: Nausea may occur. If this effect persists or worsens, contact your doctor or pharmacist promptly. Tell your doctor immediately if any of these unlikely but serious side effects occur: drowsiness, mental/mood changes, ringing in the ears, dizziness, vision changes, tremors, numbness, headache, backache. Tell your doctor immediately if any of these highly unlikely but very serious side effects occur: fever, unusually fast or slow pulse, trouble breathing, seizures, chest pain. An allergic reaction to this drug is unlikely, but seek immediate medical attention if it occurs. Symptoms of an allergic reaction include: rash, itching, swelling, dizziness, trouble breathing. If you notice other effects not listed above, contact your doctor or pharmacist.

PRECAUTIONS: Tell your doctor your medical history, especially: heart problems (e.g., heart block, heart failure), high or low blood pressure, liver problems, kidney problems, any allergies. This medication is not recommended for use if you have the following medical conditions: nerve disease, spine problems. This drug may make you dizzy or drowsy; use caution engaging in activities requiring alertness such as driving or using machinery. Avoid alcoholic beverages. Depending on how and where this drug is injected into the body, you may experience temporary weakness. To minimize dizziness and lightheadedness, get up slowly when rising from a seated or lying position. Notify your doctor if weakness or problems with muscle control persist. Caution is advised when using this drug in the elderly because they may be more sensitive to the effects of the drug. Caution is advised when using this drug in children because they may be more sensitive to the effects the drug. Tell your doctor if you are pregnant before using this medication. This medication passes into breast milk. Consult your doctor before breast-feeding.

DRUG INTERACTIONS: This drug is not recommended for use with: dofetilide, arbutamine, pimozide, halofantrine. Tell your doctor or pharmacist of all prescription and nonprescription drugs you may use, especially: other anesthetics (e.g., bupivacaine), procainamide, tocainide, cimetidine, beta- blockers (e.g., propranolol, atenolol, metoprolol), amiodarone, mexilitine, succinylcholine, vasopressor drugs (e.g., dobutamine), ergot-type oxytocic (e.g., ergonovine). Tell your doctor if you take drugs that cause drowsiness such as: medicine for sleep, sedatives, tranquilizers, anti-anxiety drugs (e.g., diazepam), narcotic pain relievers (e.g., codeine), psychiatric medicines (e.g., phenothiazines or tricyclics), anti- seizure drugs (e.g., carbamazepine), muscle relaxants, antihistamines that cause drowsiness (e.g., diphenhydramine). Check the labels on all your medicines (e.g., cough-and-cold products) because they may contain drowsiness-causing ingredients. Ask your pharmacist about the safe use of those products. This product can interfere with certain lab tests. Inform laboratory personnel that you are using this medication. Do not start or stop any medicine without doctor or pharmacist approval.

OVERDOSE: If overdose is suspected, contact your local poison control center or emergency room immediately. US residents can call the US national poison hotline at 1-800-222-1222. Canadian residents should call their local poison control center directly. Symptoms of overdose may include restlessness, anxiety, ringing in the ears, blurred vision, tremors; or severe dizziness or drowsiness.

NOTES: Do not share this medication with others. Laboratory and/or medical tests may be performed to monitor for side effects and response to treatment.

MISSED DOSE: If the dose is interrupted, contact your doctor or pharmacist immediately to establish new dosing.

STORAGE: Store at room temperature between 59 and 77 degrees F (15 to 25 degrees C) away from light and moisture. Discard any unused liquid.
